Overview
Get response URLs and download QR codes from Microsoft Forms for sharing surveys and quizzes.
URLs

Critical Constraints
- Authentication Required: Must be logged into Microsoft 365 account
- Hidden QR Tab: QR code tab only appears on hover near the link area
- Shorten URL: Recommended for print materials - creates cleaner URLs
Workflow: Get Share URL and QR Code
1. Navigate to Form Edit Page
Navigate to your form's edit page:
https://forms.office.com/Pages/DesignPageV2.aspx?subpage=design&FormId=...
2. Open Share Dialog
- Find the "Collect responses" button in the toolbar
- Click to open the share dialog
3. Configure Share Settings
- Select who can respond (Anyone / Organization only / Specific people)
- Toggle "Shorten URL" checkbox if you want a shorter link
4. Copy Response URL
- Find the textbox labeled "Link for Collecting Responses"
- Copy the URL displayed (format:
https://forms.office.com/r/XXXXXXXXif shortened)
5. Download QR Code
- Hover near the "Copy link" button to reveal hidden tabs
- Click the "QR code" tab that appears
- Click the "Download" button
- QR code PNG file is saved to downloads folder

Troubleshooting

Notes
- QR code filename includes form title with special characters replaced by hyphens
- Shortened URLs are recommended for printed materials and presentations
- QR codes encode the response URL, not the edit URL
